The knowledgeable and experienced staff at your local Modern Dental Center will work directly with you to develop a detailed treatment plan for your case. Our team of experts will continue to support you throughout the entire process to ensure that the final restoration exceeds your expectations.
Traditional impressions or model case:
- Download the prescription form (RX form) <link>, fill in the form with instructions for your case
- Call or email us to request a Shipping Kit, which includes a case box, shipping bag, mailing label, and the contact information of our preferred pick-up / delivery courier.
- The Shipping Kit will be delivered to your office usually the next business day, depending on your location.
- When you’re ready, call our preferred pick-up courier to pick up your case. Make sure to include the completed prescription form (Rx) and the full set of the shipping kit.
Digital impressions:
- Call us to set-up your account.
- Fill in the prescription form (Rx form) with instructions for your case.
- Upload your prescription (RX form) to our website once your account has been activated.
- Digital cases: 7 business days
- Physical impressions: 12-14 business days
- To initiate a remake, please return the original dental restoration to us.
- If the product is damaged, please send back any part of the device that is available.
- If the product is lost, please contact us to trace the history of the corresponding products. Invoice will be issued until product is returned. If new impression is required, and you decide to proceed without taking a new impression, we will proceed according to your instructions, but the remake policy will be null and void. Any future remakes will be charged at full cost.
What is covered?
Our warranty program applies only to products ordered through our Canada lab and not to products from other Modern Dental locations. We guarantee to repair or remake any defects caused by faulty materials or workmanship at no additional charge.
What is not covered?
The warranty program does not cover custom-made restorations or defects caused by accident, negligence, abuse, failure of supportive tooth structure or tissue structures, improper adjustments, or poor dental hygiene. We do not offer cash refunds or reimbursement for costs associated with removal or insertion.
