Modern Remake Policy

We are pleased to offer an easy and in-depth remake policy for Doctors. The policy includes all restorations or appliances that are covered by our warranty program and fabricated according to your specification and approval.

If there are any issues with a restoration or appliance during the warranty period, we will remake or repair the restoration or appliance.

How our policy works:

1. The original dental restoration must be returned

  • If the product is damaged, please send any part of it available.
  • If the product is lost, please contact us.
  • A return deposit will be invoiced, until the product is returned.
  • If the product was being used as a temporary, simply return it and the deposit will be credited

2. If the laboratory requests a new impression, but the Doctor instructs to proceed without a new impression, the remake policy is null and void. Any further remake will be completed at full price.

  1. Product warranty period refers to the case of lab reason cause remake. If it is due to a doctor’s reason cause remake or modification, there is no warranty 
  2. Ortho appliance, bruxism, and snoreguard: 3 months to 6 months, subject to the function or appliance product beyond their liftetime 
  3. Temporary crown/bridge or temporary/immediate denture without warranty period 
  4. Consumable parts, no warranty. For example: ATT Parts, implant parts, etc

What does it cover?

Work is guaranteed to be free of defects due to materials and workmanship. The appliance is guaranteed to fit the provided model and to be constructed to the design requested on the prescription form. Appliance will be repaired or remade at no charge if the workmanship or material is causing problems. The warranty runs starting from the date the case is received at our laboratory.

What is not covered?

  • Cash refunds for a custom made restoration
  • Cost incurred for removal or insertion
  • Repairs resulting from accident, neglect, abuse, failure of supportive tooth structure or tissue structures
  • Improper adjustments or dental hygiene
  • Prosthesis partially or completely fabricated by another lab other than Modern Dental Laboratory Canada.

The liability for products invoiced by Modern Dental Laboratory Canada is strictly limited to those products.

Billing

Credit Policy – The initial credit limit is $1500, until credit is established. Credit can be established at Modern’s sole discretion through the use of our New Account Application or history with Modern Dental Laboratory Canada.

Billing – We have two methods of informing you of the amount due:

  • Invoice – You will receive an invoice with every case delivered to you, which details the products and associated fees incurred.
  • Statement – You will be mailed a statement of outstanding invoices at the end of every month.
  • Payment of the outstanding balance is due within 25 days of the statement date.

Overdue Amounts – A finance charge of 1.5% per month of the outstanding balance (whichever amount is greater) will be posted to any accounts that are past due. Accounts with a past due of 25 days past the statement date may be put on hold and sent to collections.

Late payments can result in interruption of service.

Modern Dental Laboratory Canada reserves the rights to place accounts on hold for non-payment.

Modern Dental Laboratory Canada reserves the rights to refuse service for any reason.

Payment Methods – We accept two methods of payment: Checks and Credit Cards. We accept Visa, MasterCard, Discover and American Express.
